This form is for dates that you would like to be closed in Polaris. So that fines and due dates do not apply to your patrons. If you'd like to inform delivery of dates that you don't want delivery, please fill out the No Delivery Request Form.

If your closed dates apply to both Polaris and Delivery please copy and fill out both forms.
